Name Tag Form

Your name tag signifies you are a student at the KU School of Pharmacy. You will use it throughout your four-year tenure at School of Pharmacy. Complete the KU Pharmacy P1 Name Tag Form to order your complementary name tag.

FERPA Consent Form

Please complete the FERPA Consent Form.

Video Release Form

Please complete the KU School of Pharmacy Video Release Form.

Photo Release Form

Please complete the KU School of Pharmacy Photo Release Form.

iPads and Required Apps

All students entering KU School of Pharmacy’s Pharm.D. program are required to own an iPad, as well as select accessories and applications. iPads are used in the classroom and during experiential rotations in a variety of ways including exam administration.
