Pharmacy Intern Registration Requirements


Matriculation into the KU School of Pharmacy requires that you become registered as a Pharmacy Intern with the Kansas State Board of Pharmacy. Registration Process

Application Fees

  • Applicants that DO NOT need a background check are those with an active Kansas Rx Technician registration – Total application cost $20
  • Applicants that DO need a background check (no active Kansas Rx Technician registration) – Total application cost $77 ($20 for the application + $57 for the KBI background check).

PDF Forms Must Be Submitted by U.S. Mail
Printable PDF forms [LA-03 (application), S-100 (background checks), and S-150 (conditional personal history form)] must be printed and mailed to the Kansas Board of Pharmacy at 800 SW Jackson, Suite 1414, Topeka, Kansas 66612-1244. Do not email any of the forms. Confidential information should never be transmitted by email.

Conditional Forms

Not required by all - you will be notified by the Dean's Office via email regarding whether you are required to complete FD-258 and S-100 (background check).

Please follow noted instructions below for Form S-150 - if required.

  • Kansas Board of Pharmacy - FD-258 FINGERPRINT CARD ORDER FORM
    If required, complete this ASAP - The Kansas Board of Pharmacy will determine if you are required to submit fingerprints. If required, form FD-258 can be downloaded and printed (white stock paper only), mailed to you directly, or local police and sheriff departments typically maintain fingerprint cards. You will be notified by the KU School of Pharmacy regarding whether you DO or DO NOT require fingerprinting (a small fee may apply).
  • KBI/FBI Criminal Background Check Form (Form: S-100)
    If required, complete this ASAP - A criminal background check will be conducted through the Kansas Bureau of Investigation and the Federal Bureau of Investigation.
  • Registration Application – Personal History (Form: S-150)
    If you answered "Yes" to any questions on the Personal History Information section (page 3 of form LA-03) you will be required to also submit a supplemental Personal History form S-150.

Intern Registration Number and Card

Upon successful application, the Kansas State Board of Pharmacy will assign you a Pharmacy Intern registration number and card which will be forwarded to the KU School of Pharmacy for distribution during the fall semester. The card will reflect an effective date (the first day of class) and an expiration date. You should have your Pharmacy Intern registration card in your possession when working in a pharmacy practice environment or engaged in pharmacy student patient-related activities. Your Pharmacy Intern registration with the Kansas Board of Pharmacy provides Pharmacy Intern status ONLY in the state of Kansas.
